In 250 words or less, tell us how your DFF, used both compassion and creativity to help you (or a loved one) understand, manage and live with diabetes.
- Nominees will be judged on their compassion (70%), and creativity (30%) in helping to educate and motivate the person with diabetes to better manage their diabetes. Specific examples of their efforts must be cited.
- Entrants may nominate more than one DFF, but each nomination must be a separate entry.

- On the following confirmation page click the "Submit Nomination Video" button.
- You will be taken to YouTube and must click "Allow Access" to allow your video to be uploaded. You may need to log in to YouTube first.
- Click the "Choose File" button, select your video.
- Click "Upload Video File" and remain on the page until told the upload has finished.

Many of us have a BFF (Best Friend Forever)
that we look to for laughs, encouragement and support. But those with diabetes or pre-diabetes
also need someone who can be a friend and understand a diabetic lifestyle. That’s what
we call a DFF – Diabetes Friend Forever.DFFs are friends, family members, caregivers and healthcare professionals who make a difference in the lives of those living with diabetes. They are teachers, counselors, coaches, and best friends to some of the 104 million Americans who have diabetes or pre-diabetes today.
To honor – and learn from -- these unsung heroes, Dreamfields Pasta is launching a fun, interactive program to help pay tribute to the special people who help make living with diabetes a manageable experience.
